CASE MANAGEMENT

 
 

WHAT IS CASE MANAGEMENT:

Case management is not a “one size fits all” service. Case management can be transformed into whatever you need as the client. Primarily, here at RPC, case management is divided up into three main types of service:


Therapeutic Interventions 

Case managers are safe spaces who can assist you with therapeutic treatment goals outside of therapy. We are able to assist you in implementing treatment goals that are sometimes difficult to carry out on your own. Some examples of this are: implementing money management plans by going grocery shopping together, overcoming social anxiety by going to breakfast at a new place together, or promoting socialization by attending an event outside of the house together. 


Advocacy 

Case managers assist clients in advocating for themselves when it comes to a variety of issues. For instance, we can attend doctor’s appointments with you to coordinate care with your primary care physician or psychiatrist. We can assist in communicating your current symptoms and help you keep track of diagnosis and care instructions. We can help by researching providers in your area to find the best fit for your medical needs. It could also include advocating for your needs for services from community resources in your area.


Linkage to Community Resources 

Case managers are the central point when it comes to connecting you as the individual with community resources that are available in your area. Some of the main resources case managers link their clients to include: housing assistance, food assistance, insurance coverage, income based medical care, transportation, job training/education, social security/disability applications, legal aid, child care assistance, etc.

How is Case Management Helpful?

Case management is helpful because when you are struggling with meeting your basic needs like housing, food, stable income, etc., you lack an important sense of safety and security. It is difficult for anyone to see past a lack of basic needs to focus on in-depth, intense treatment with a clinician. Until that sense of safety and security has been met, it will be hard to make progress in therapy. Case managers assist you by helping you reach a place of safety and security by linking you to the right community resources to get back on your feet. 

Case managers are also helpful in implementing treatment goals and therapy interventions into your day to day life. Some of us struggle taking action on our treatment goals because we don’t feel as though we have a safe space to try outside of the office. Case managers can help you get out of your comfort zone and assist on putting into place therapeutic techniques in your home and community. This allows case managers to be an extension of the treatment you are doing with your clinician in the office.
